Terms and Conditions for Equipment Loan
Declaration
When you book equipment through the SISO service you are deemed to have confirmed the following:
· I confirm that I have discussed and agreed with my academic supervisor/Line Manager all aspects of the activity, and have drawn particular attention to any situation in which people or equipment may be put at risk.
· I confirm that I have informed all participants in the activity of the above matters, and that I have taken all reasonable precautions to ensure the safety of those participating in the activity.
· I confirm that I have obtained all necessary clearances my project, that all locations are detailed on the Risk Assessment and that I have discussed any likely problems with my academic supervisor.
· A Risk Assessment has been lodged with the School.
· I confirm that during my project I will work in a professional and responsible manner, following established procedures and following the instructions given during demonstrations and training.
· I understand that all Equipment borrowed will be subject to the Terms and Conditions for Equipment Loan set out below.

2. Risk assessment
A Risk Assessment must be undertaken in accordance with Bournemouth
University's Health and Safety Policy and to comply with legal requirements. Risk assessment forms can be found in the
Health and Safety section of the %u201CFind a Form%u201D page on the intranet: Find a Form
- Staff undertaking Risk Assessment must submit the hired Risk Assessment
document to the Administration team (C237), and obtain the Risk Assessment
reference number. The reference number must be entered onto the SISO request.
- Students undertaking Risk Assessment must submit the document to their academic
supervisor who will assume overall responsibility for the project, and will not
authorise your SiSo request until satisfied that an appropriate Risk Assessment
has been carried out and a safe system of work devised and agreed. All aspects
of the operation must be discussed between the academic supervisor and the
user. This may include matters such as misuse of equipment, lone working, trip
and electrical hazards etc.
- For on-campus demonstration purposes, a standard risk assessment is available
from the Resources Officer, which should be obtained and registered on SISO.
Where identified by your academic supervisor, you will be responsible for
advising the local authorities of any activity highlighted as risk to general
public or public areas.
3. Overseas Travel and Fieldwork
All equipment to be taken overseas (to include fieldwork, research and student
travel) must have written Dean / DOps approval prior to booking. Staff should
complete a Carnet, and request signed approval from their line manager. A PDF
version of the signed Carnet should be uploaded onto the SISO request and
logged on file with the risk assessment and travel approval form in C237.
The original Carnet should be carried at all times during the travel for
insurance purposes and logged with the University's insurance officer.
4. Training
Staff may train students to use specific equipment available via SiSo. Staff
must ensure a Training Register is completed. Forms are available from the
Resources Officer.
Students who receive training for specific equipment must ensure they sign the
Training Register on completion of the training. The training register will be
uploaded onto SiSo. Only trained individuals will be permitted to use the
specified equipment.
5. Specialist Items
Where an item or a group of items constitutes specialist kit, these items will
be identified and logged on SISO as available to specific, trained staff.
Requests for these items will be granted only to those staff who are identified
on SISO as having permission to borrow the identified kit.
6. Research Projects (IRP/DRP) and Placements
Student research projects (IRP/DRP) require completion of the Application for
the use of Field Equipment, Laboratory Equipment, Laboratory Facilities and
Support available through the Resources Officer. Student placements where equipment
will be requested via SiSo, require completion of an Application for use of
Field Equipment while on Placement available through the Resources Officer .
Forms should be completed and returned to the Resources Officer within the
School. Once approved, any equipment should be requested via SiSo in accordance
with these terms and conditions.
7. Non collection of equipment
Equipment booked for collection will only be kept for up to 48 hours, after
which your booking will be cancelled and you will have to re-book.
8. Equipment lost or stolen
Any loss or theft of equipment must be reported immediately to the Resources
Officer. You will be required to provide a written report, providing full
details of the incident. You must report any theft of equipment to the Police
in the area where the theft is thought to have occurred and obtain a police
report number. When requested, you must complete an insurance claim form. Losses or thefts will be investigated and you
may be required to contribute or pay the full cost of replacement.
9. Equipment returned damaged
If you return damaged Equipment you must provide a written report to the
Resources Officer. Where damage appears to have been caused deliberately
or by negligence o this will be referred
to the Dean / DOps. When requested, you must complete an insurance claim form
in the time frames requested. Damage will
be investigated and you may be required to contribute or pay the full cost of repair
or replacement.
10. Overdue Equipment - Student
Scheduling of equipment is critical, and the success of the system relies on
the timely return of borrowed resource. Failure to return borrowed equipment
will mean other students or staff will not be able to complete assignments. Equipment are available for
a maximum term of 14 days. Any request for a loan for a longer period must be
approved via your supervisor and Director of Operations.
If, for any reason, you are delayed in returning equipment on the due date,
please contact the Resources Officer as soon as possible. It may be possible to
extend your booking, or make alternative arrangements for equipment to be made
available to the next user.
When a resource is not returned on the due date, an overdue reminder will be
sent by email to your University email account. Subsequent computer-generated
reminders are issued every 48hrs, subject to system availability. It remains
your responsibility to return items on time or be liable for the late return
fees, whether or not the overdue reminders are sent or received. Please ensure
you access your University email account regularly.
Overdue fees will be charged for late return of equipment at a rate of
£2 per day or such other rate as the Resources Officer may advise from time to
time.
Fees will be incurred for overdue equipment at a daily rate up to and
including the day of return. If you have overdue equipment you will not be
entitled to borrow anything else until those equipment are returned and the
associated late return fees are paid. When a late return fee is incurred, the
relevant SISO account will be blocked, and you will not be able to borrow any
further equipment.

regarding an overdue account, please contact the Resources Officer.
Your attention is drawn to the Student Fees Policy in force from time to time (available on the intranet) which sets out the University policy relating to late payment of charges and the sanctions which may be applied. Students may be denied access to specific University facilities or an award (if eligible) until all fees other charges are paid in full.
11. Overdue Equipment - Staff
Scheduling of equipment is critical, and the success of the system relies on
the timely return of borrowed equipment. Failure to return equipment will deny
other Students or Staff expecting booked equipment to be available.
Equipment are available for a maximum term of 14 days. Any request for a
loan for a longer period must be approved via the Director of Operations.
An overdue reminder will be sent by email to your University account
when the equipment is overdue. If you have overdue equipment you will not be
entitled to borrow further equipment until the equipment are returned.

system availability. It remains your responsibility to return equipment on
time, whether or not the overdue reminders are sent or received.
Staff SISO accounts will be blocked until kit is returned, and further
requests for loans will not be possible.

12. Responsibility for Equipment
You are responsible for all equipment borrowed, as itemised in the SiSo request
and must ensure the equipment are returned to the Store. Equipment must not be
sub-let by you, or directly passed to another member of staff or student.
Passing equipment directly to another student or colleague will not be seen as
a proper handover of responsibility and you will remain fully responsible for proper and
timely return of the equipment.
13. Care of Equipment
Always be careful when using University equipment. Never leave equipment
unattended when in use and always store equipment away safely and securely at
other times. Take care to avoid damaging equipment, particularly with food and
drink, extreme weather conditions and detrimental environments.
All borrowed equipment must be returned in the same state as issued. You must
ensure all equipment is clean before returning it.
14. Use of your personal data
Data that you input into this system may be shared with third parties to enable provision of this service. For more information please see the University policy for Data Protection, which is available on the intranet.
15. SiSo terms and conditions of use
As a user of this system registered or otherwise you agree that if for any
reason you are unable to use SiSo for the intended purpose then you will
contact your store via telephone, email or in person to resolve the problem.

or any other loss caused by your usage of this system. For the avoidance of
doubt this includes any loss howsoever suffered including negligence or fraud
except where such negligence causes death or personal injury.
